Course Material Shipment Form
Only fill out this form if you require your distance education course materials sent to you by courier. Reminder: If you are living or taking courses on campus, you can pick up your course materials in person at the times & location noted here.
This form is NOT for ordering textbooks it is only for ordering your supplied course materials by shipment.
If you are unable to come to campus to pick up your course materials, please fill out the online shipping form below and submit it as soon as possible. If you do not fill out the online shipping form, we will assume that you are coming to the University to pick up your materials.
Shipment of supplied course materials for SUMMER 2013 DE commences Monday, April 8, 2013. Please allow 1 to 3 days for delivery within Canada and up to a week for US and International shipments. Please note that the courier company requires a signature** for the delivery of packages, and only delivers Monday through Friday from 8:00 a.m. to 5:00 p.m. If you are not home during these hours, please give us an alternative address for the courier, such as your business address or another address where someone can sign for your materials. **P.O. Box numbers are un-acceptable as a material delivery address.
Note: Address information provided for shipping will not be used for returning assignments. To permanently change your address, please complete the Address Change form.
IMPORTANT NOTE: Choose the correct semester below that you are registered in ie) Summer 2013 or Winter 2013 before you select the course code for materials to ship.
