Final Examination Information Form

NOTE: This form is for courses taken in the Winter 2012 semester only!!

It is assumed that all students will be writing their final exam(s) at the University of Guelph. However, arrangements can be made for alternate locations, listed below, provided the exam form is received by Monday, February 6th, 2012.

Please note that a $30 late fee will apply if:

  • You submit this form after the deadline date as this will result in a change in your exam location
  • You request a change in your exam location after the deadline date.  This includes changing your location back to Guelph if you have requested to write off-campus.

If your address is in Canada, we will make arrangements for you to write your final exam at an educational institution near you. One month prior to the examination period you will be mailed details of your final examination date, time and location based on your selection and completion of this form. Your final examination will not necessarily be scheduled on the same day as the on-campus exam and you must be available to write on any date and time during the scheduled exam period. Please submit the completed form by February 6th, 2012.

If you are requesting to write your exam outside of Canada, please note that you will be responsible for securing your own proctor at an approved exam site and will be responsible for paying any exam related fees.  More information will be emailed to you in early February.

Important Note: Students indicating they are registered with the CSD who require additional exam accommodations must be registered with the CSD for the current semester. 

Exam location changes cannot be made after February 27th, 2012.
